I would imagine the normal route is for the council to advise the driver a complaint has been made and they invite him to a meeting.
IMO they should give him a rough outline of the complaint, which would allow him to arrange his side of the story, including any witnesses.
If they don't then he should go to the meeting and simply refuse to comment until he has taken advice from a union/association/CAB.
As for the answer to question 3, TBH I haven't got a clue what you are asking.
